• Application for Sanction (from National site)

Optional documents for race sanctions (available on National site):

• Request for Certificate of Liability Insurance (if requested by sponsor, etc.)
• Elite Sanctions Addendum (if you give award $$ or invite elite athletes)
• Waiver of Insurance form (if you only need sanction but are providing alternative insurance)
